Volunteer Clearances

If you are 18 years-of-age or older and are interested in volunteering for the DME retreat program, you will need to fill out the clearances below. In accordance with the laws of the Commonwealth of Pennsylvania and the designation of our volunteers as "regular" rather than "occasional", all of our volunteers complete the following clearances. All paperwork for clearances are kept on file with the retreat program. All record checks need to have been completed after July 2015 and thereafter every five years as indicated. Volunteers should also keep a copy of these files for their own records.

Participate in the 51 minute training video and take the online quiz

Print your certificate and keep a copy for your files

To be completed every five years as of July 2015 (No dates prior to this will be accepted)

Complete the form and submit - your clearance will either be available immediately, will be emailed to you, or you will be able to check back with your registration number.

To be completed every five years as of July 2015. (No dates prior to this will be accepted)

Complete and submit - a copy will be available immediately, will be mailed to you, or you will be able to check back in with your registration number.

FBI Record Check & Fingerprinting

Must be completed by anyone that has not lived continuously in Pennsylvania for the past decade.
