Volunteer Clearances

If you are 18 years-of-age or older and are interested in volunteering for the DME retreat program, you will need to fill out the clearances below. In accordance with the laws of the Commonwealth of Pennsylvania and the designation of our volunteers as "non-school regular volunteer" rather than "occasional", all of our volunteers complete the following clearances. All paperwork for clearances are kept on file with the retreat program. Volunteers should also keep a copy of these files for their own records.

PA CRIMINAL RECORD CHECK

To be completed every five years.

​

Complete the form and submit - your clearance will either be available immediately, will be emailed to you, or you will be able to check back with your registration number.

PA DEPARTMENT OF HUMAN SERVICES CHILD ABUSE RECORD CHECK

To be completed every five years.

Complete and submit - a copy will be available immediately, will be mailed to you, or you will be able to check back in with your registration number.

DIOCESE OF ERIE CREATING A SAFE ENVIRONMENT ONLINE TRAINING

To be completed every five years.

​

Participate in the 51 minute training video and pass the online quiz with a score of at least 80%. 

Print your certificate and keep a copy for your files.

DIOCESE OF ERIE APPLICATION FOR ADULTS WORKING WITH YOUTH

To be completed once after 7/1/2018.

Every line must be filled out, with the exception of the office use and supervisor portion. If a question does not apply to you, simply write "n/a" on the line.
